      Provide text-entry options for each item (tabble, file, URL). For example, user right-clicks on a document or URL or Tabble, and selects "edit notes."

      Allow user-specified names for the text-entry boxes. For example, a group of 3 people, A, B, and C, are all reviewing documents.
      They each want their own "notes" field. A notes, B notes, etc.
      This could be done at the Tabble level such that all items within the Tabble (and sub-Tabbles) get the same text-entry template.
